Common Reporting Errors

Four common problems with reporting:

  1. Total number of unduplicated clients exceeds total visits: The total number of visits should be greater than the total number of unduplicated clients.
  2. Clients by age data are not reported: These numbers are used in the Body Mass Index (BMI) measure calculation. No data will appear in your report for this measure if these data are missing.
  3. The total number of clients by age and the total number of unduplicated clients served differed significantly: While this discrepancy will not affect the measure calculations, it may mean the denominator for the BMI measure is under/over-reported if the clients by age data are incorrect.
  4. Data reported were missing either the numerator or denominator needed for performance measure calculations (partial data) or resulted in percentages that exceeded 100% (indicating possible duplicate client counts).
